Managing Resident Cap

Germain Solutions will work closely with DIOs to restructure the number of resident assigned to a program in ways that will allow the program to stay under the resident cap while improving the quality of the resident experience.

Analysis of Physician Activities

Using a variety of interactive tools (see Technology Tab) Germain will help to determine a physicians productivity in a variety of areas including billing, coding, RVU productivity, teaching, and scheduling. The idea is if physicians can improve in one or all of these areas they will be more valuable to the hospital and provide a better overall experience for their patients.

Physician Scheduling

The majority of patient care problem that occur in hospitals are a direct result of poor scheduling and every mistake cost the hospital thousands in lost revenue and subjects them to a growing malpractice liability. Germain provides specialized resources that allow the person in charge of scheduling to schedule all physicians without the fear of overlapping and forgetting areas of need. Further the tool guides the scheduler to scheduling the appropriate amount of days per physician in order to maximize cost effectiveness for the program.

Leverage Investment of Academic Infrastructure

One of the most complex and complicated aspects of GME, Germain will work with DIOs to leverage academic infrastructure in ways that are most cost effective for the program. Solutions such as adjusting faculty to resident ratios and the inclusion of less costly support staff often lead to considerable cost savings within teaching programs.

Redesign of Teaching Programs

Certain programs would benefit from a complete redesign of their current operation structure. Although extensive work is required, a redesign of a program often opens it up to lucrative opportunities both within the hospital and with other outside partners:

  • Adjust Program Size and Mix
  • Faculty/Community/Physician Realignment
  • Partnerships – Community Organizations (FQHC, etc.)
  • Medical School Alignment & Integration

Germain’s primary objective when interacting with program directors is to determine if the current operational structure is “the right fit” for the program. If it is not, Germain is committed to working with the program directors to restructure the program so it will be effective. Areas to be determined include:

  • Is there is a need to establish new programs either within the hospital or in conjunction with another institution?
  • Is there a benefit to being over/under the Medicare Cap?
  • Should the residency program need to be eliminated?

Resident Rotation Schedule

Germain can assess the current resident rotation schedule and provide recommendations on how to alter the current structure in ways that will enhance the education experience.
